How should an intro be made and should I use one of the MIDI files from Vlarion- The Elemental War?

I don't think that's an important rule.

However, a better rule is never use .MIDI songs.

It's up to you. If it's a horror movie, show the title accompanied by scary music. If it's a comedy, have the title in silly lettering with goofy music. If it's a drama, use a normal looking font and have serious sounding music. You get the idea.

Intros shouldn't really be that long or complicated anyway.

I’d just recommend watching actual movies or 3D movies and trying to emulate that style.
